Aim of the Centre
The Centre is working with local statutory bodies, the voluntary sector, the League of Friends of Fenwick2 and various professional bodies to develop this website, and this is an ongoing process.
The Centre aims to develop a strong focus on meeting patients and carer information needs in the locality. The centre is delivering its services in partnership with Carers Together in Hampshire.
The aims of the PACRC are to:
a) Ensure that the centre becomes a respected hub for local patient information delivery, access to resources and support through trained volunteers.
b) To give carers information, support and advice to enable them to care more effectively and safeguard their own well-being;
c) To encourage self help among patients by supplying information, advice and support concerning their condition;
d) To provide good quality of signposting of services depending on patients and carers needs.
We are developing a strong focus on supporting people with long term conditions, linking to the Hampshire Primary Care Trust Long Term Condition Centre and the specialist nurses and services located at Fenwick2.
The centre can also assist with access to programmes such as the Expert Patients Programme and Carers Together in Hampshire carer training and awareness programmes. We have good working relationships with other statutory and voluntary organisations.
Local Point of Access and appropriate information
Drop in Resource Centre
The Centre is manned by trained volunteers who have been through a comprehensive training programme provided by the SCA group. The Centre is able to sign post to local services, appropriate websites and respond to written enquiries.
Staff working with our team of appropriately trained volunteers are available to explain information and this can be followed up with written material to take away. There is a clearly defined protocol explaining what the centre can and cannot do.
Telephone help line
As well as functioning as a local drop-in recourse centre, we are able to provide a telephone help-line, during some of the working hours each week. This help line number is 023 8028 6341.
When the Centre is closed you can leave an answer phone message.
Appropriate Information
The Resource Room is situated in a pleasant, relaxed and informal supportive environment and is located within the Fenwick Health and Well Being Centre adjacent to the Carers Together in Hampshire, New Forest Office.
The Patients' and Carers' Resource room is equipped with a table and chairs, and a desk with a computer for using the internet and sending e-mails.
Information is available in different media, with a strong focus on ‘on line' information, in order to meet the requirements of all potential users. Although we aim to provide information which is easy to understand, staff can help explain anything which is not immediately understood. We will provide information about local health and social care services and how to access them. This includes accessing information on treatment; other service providers, self-help and support groups, advocacy and benefits.
