SCA pioneered the Welcome Home service over a decade ago, in order to give short term support or personal care for a time limited period on discharge from hospital. This service is popular with both service users and their relatives as it enables the newly discharged patient to regain their confidence without creating long term dependency.
It is also popular with Commissioners and Purchasers as it releases hospital beds and offers some financial savings by ensuring a service is not delivered indefinitely when it is no longer needed.
This service operates in :
Havant, Petersfield, Fareham, Gosport (Waterlooville Office)
Romsey, Eastleigh, Southampton (for Orthopaedic cases only) (Southampton Office)
New Forest areas (New Forest office)
It is also available via local Adult Services.
