This short term service is available to residents in Poole (Rehabilitation Homecare) who are being discharged from any hospital. It aims to provide up to six weeks of rehabilitation support following discharge from hospital, but with the addition of a personalised rehabilitation programme.
The programme includes a series of exercises that will maximise the strength and confidence of the service user, enabling them to return to their previous level of independence, or to a level whereby minimal ongoing support is required. SCA has its base in the centre of the Woodland Rehabilitation Unit at Alderney Community Hospital in Poole, which allows for the flow of information between our rehabilitation homecare workers, and the Physiotherapists and Occupational Therapists, which in turn improves efficiency in delivery care to the service user.
Access to this service is usually via the brokerage team at Poole Adult Services.
