Reablement
SCA Care provides reablement services to support customers to recuperate in their own homes following a stay in hospital. These services are called our Welcome Home service and Rehabilitation Homecare Service.
Last year we supported 1689 people with their discharge from hospital and recuperation in the comfort of their own home. Of these customers, 75% were enabled to become fully independent again and only 8% were readmitted to hospital, for our customer groups, this is a positive reduction to half the national average. Last year, our rehabilitation services saved the NHS over £2million, and our high-reablement rates significantly reduce costs to adult social services.

POOLE REHABILITATION HOMECARE SERVICE:
Our Poole Rehabilitation Homecare service is delivered in partnership with Poole Borough Council and Bournemouth & Poole PCT. It supports residents of Poole who are being discharged from any hospital. It provides up to six weeks of rehabilitation support following discharge from hospital, but with the addition of a personalised rehabilitation programme.
The programme includes a series of exercises that will maximise the strength and confidence of the customer, enabling them to return to their previous level of independence, or to a level whereby minimal ongoing support is required.
Access to this service is usually via the brokerage team at Poole Adult Services however we can deliver this service through a private agreement if required. Please call our Poole office for more information:

WELCOME HOME:
Our Welcome Home services operate from our Southampton, East Hampshire and New Forest SAC Care offices. Similarly to the Poole Rehabilitation Homecare Service, the Welcome Home service provides time limited care and support following a stay in hospital. The service enables the customer to recuperate in their own home, gain confidence and regain independence.
The Welcome Home services are available through adult social services.
