Extra Care

SCA provides specialist teams of care workers to a number of Extra Care Housing Schemes in Southampton and Dorset.

Extra Care Housing is a type of sheltered housing that is designed to meet the needs of frail and vulnerable older people. It enables them to maximise their independence and continue to remain living in their community as their support needs increase, and thus reduce the need for residential care.

By supplying care workers who deliver personal care, shopping, domestic and companion services as a single package of care, care would be more 'joined up' for the customer, and prevent admission to residential care for as long as possible.

The objectives of Extra Care are as follows:

• Diversity and choice in the provision of housing support and care.
• Service user's needs and any increased needs are supported by a flexible responsive service.
• Service users remain as independent as possible within their local community.
• Service users overall sense of wellbeing is improved and their dignity respected by remaining in their own home.
• Service users access the wider community beyond the housing unit.

EXTRA CARE HOUSING IN DORSET

SCA delivers the Extra Care service at Trailway Court, Blandford Dorset. This is an innovative and pioneering service which embraces all aspects of the Transforming Social Care Agenda by using local services as much as possible and encouraging the wider community to share the resources. Locally sourced staff have used their local knowledge and worked with Synergy's Scheme Manager to link in with local community projects and maximise the promotion of wellbeing and fulfilment for each tenant.

SCA Care is working in partnership with Synergy Housing, North Dorset County Council, the Reablement Team, Careline and Dorset PCT. This partnership is committed to achieving the eight outcomes for older people as identified in Dorset's Ageing Well (2009).
